Pain Management*
We offer a telephone-based holistic, body-mind treatment designed to complement the medical management of chronic pain conditions as well as the negative emotions that almost always accompany them. Many of our members have found relief from arthritis pain, back pain, fibromyalgia, joint pain, lupus, migraine and tension headache, multiple sclerosis, neck pain and other chronic pain conditions through this service. Disease Management
Our contracted health plans offer disease management programs to help you better manage a health condition. Program availability varies by health plan but may include diabetes, asthma, heart disease and other related heart and circulatory conditions, low back pain, smoking cessation, depression, prenatal care, neonatal care and cancer. Case Management
Through our innovative Case Management program, we provide experienced RN case managers dedicated to working with physicians, patients and families to determine the best steps for each patient. This includes referrals to skilled nursing or specialized care facilities, community programs and more. For more information or a referral, talk with your physician.
Health Education Resources
Classes
Hill Physicians offers classes on a variety of health subjects. We work with knowledgeable instructors to provide classes in convenient locations throughout the bay area, greater Sacramento area and San Joaquin county. We also offer several telephone-based classes you can participate in from the comfort of your own home or office.
Multimedia Podcasts
Our Health Multimedia section offers online podcasts on health and wellness topics from guided imagery to nutrition or relaxation. Log on any time to learn and be well.
Health Counseling*
Diabetes Education
Hill Physicians provides diabetes education with a certified diabetes educator for members who have been diagnosed with a diabetes-related condition. For a referral, talk to your physician or contact Member Health Improvement.
Health Coaching
Hill Physicians Member Health Improvement staff provides coaching to help members with certain chronic conditions better manage their health. Our coaching approach focuses on self-management through making small changes to reach reasonable and personalized goals. For more information on Health Coaching contact Member Health Improvement.
Nutrition Counseling
Hill Physicians provides nutrition counseling with a registered dietitian for members who have been diagnosed with a chronic or dietary-related condition. For a referral, talk to your physician or contact Member Health Improvement.

Complementary Medicine*
National Institutes of Health research shows complementary and alternative treatments to be useful for certain problems. Hill Physicians occasionally approves limited chiropractic or acupuncture treatments for members who do not have these benefits through their health plans.
