1. During Open Enrollment with your health plan.

If you're new to your insurance plan or completing the annual open-enrollment process, simply select Hill Physicians Medical Group on your HMO enrollment form. Your health plan may also require you to select a primary care physician.

2. You can Switch to Hill Physicians at any time.

To change medical groups, simply contact your health plan's customer service department (listed on the back of your insurance card). Ask for a doctor who is affiliated with Hill Physicians Medical Group to tap in to our network of providers and benefits.
