Step One: Be sure your HMO offers Hill Physicians Medical Group.

Step Two: Enroll as a Hill Physicians’ Member.
Whether you just joined a new health plan, or you’re looking for a new doctor, enrolling as a Hill Physicians’ Member is easy. Your insurance form may ask for your medical group preference as a separate question or at the same time you select a primary care physician. See How to Enroll here…
Step Three: Find your Primary Care Physician.
It’s probably one of the most important choices you can make for your health. Here are some things to keep in mind about How to Select a Primary Care Physician. To find a Hill Physicians’ care provider in your area use our Online Physician Directory. Our Customer Service team also is ready to help.
Step Four: Review and submit the enrollment form.
You may be able to complete this process online at your HMO’s website or by giving the completed paperwork to your HR Department or independent agent. Your HMO soon will send you an insurance card indicating your primary care physician and medical group. Here are detailed instructions on how to indicate Hill Physicians on your enrollment form...

Step Five: Schedule an Appointment with your New Provider
Once your enrollment is complete your health plan will, most likely, issue you a new card with your Providers' name on the back. However, you don't have to wait to receive that to schedule your first visit. Using the contact information found in our Physician Directory, call and schedule your first appointment.
