It’s Easy to Select a Hill Physicians Doctor for your Medicare HMO
Finding or Switching Your Provider of Choice
During the Annual Election and Open Enrollment periods you have a lot of paperwork to review. You don’t have to sift through pages of physician directories to find a physician for your Medicare plan. You can use our Online Physician Directory to find a doctor near you from our extensive network.
As long as you have chosen one of Hill Physicians’ Affiliated Medicare Advantage Plans, you may choose any Hill Physicians general practitioner as your Care Provider. And, should you decide to make a change during the year, there are easy steps to take to Switch Your Primary Care Provider at any time. If you are participating in a plan with a limited services area, our online directory can help you more easily find a doctor within the appropriate service area.
When you find the doctor of your choice, make a note of his or her name or just print out the page from our directory for your easy reference. You will want to keep that handy for your Enrollment Form paperwork.
Indicating Your Physician Choice to Your Medicare Plan
Whether you elect to enroll online or to fill out a paper form, your Medicare HMO enrollment/election form includes an area to indicate your Primary Care Provider and/or Participating Provider Group. To ensure you receive all of the benefits and services Hill Physicians’ members enjoy, be sure to indicate to your plan representative that you want Hill Physicians Medical Group.
Simply call your Medicare HMO Call Center and ask for an application to enroll as a Hill Physician Medical Group Network member and sign up with the PCP of your choice. Your Medicare HMO will take care of notifying Medicare of your affiliation with Hill Physicians to receive all of the benefits and services our members enjoy.
