If you already know that your doctor takes your Medicare Advantage Plan - you can check to see if he is with Hill Physicians, by searching below for your doctor by name:

Once you find the primary care doctor of your choice, print out their "Details" page for your easy reference. You will want to keep that handy for your Enrollment Form or to show to your Medicare Advantage Advisor.

Indicating Your Physician Choice to Your Medicare Plan

When you are Eligible to Enroll

If you have become eligible to enrolling in a Medicare Advantage HMO, it is easier when you have these items handy:

Your Medicare card or letter from Social Security or Railroad Retirement Board.
Your current health plan information, including group or Medicaid ID numbers.
Any other verification that shows you are entitled to Medicare Part A and enrolled in Medicare Part B, or enrolled in Part B only.
Your Hill Physicians' Physician Directory Details page.

1. If you are Enrolling on your own for the first time.

Whether you elect to enroll online or to fill out a paper form, your Medicare HMO enrollment form includes an area to indicate your Primary Care Provider and/or Participating Provider Group. On paper, it may look like this:

2. If you are already enrolled in a Medicare Advantage HMO.

Simply call your Medicare HMO Call Center and ask to enroll as a Hill Physician Medical Group member, and to sign up with the PCP of your choice.

3. If you are Enrolling through a Plan Representative.

To ensure you receive all of the services Hill Physicians’ members enjoy, be sure to tell your Medicare plan representative that you want Hill Physicians Medical Group.
