Switching from one Hill Physicians doctor to another is easy, and by staying within Hill Physicians Medical Group you keep your many value-added services. The process requires just a few short steps:
1. Select a New Physician

2. Contact Customer Service and Your Health Plan
Hill Physicians partners with the top HMO's in Northern California. Once you've decided on a new physician our Customer Service department will help you through the process by calling your Health Plan while you're on the phone to ensure a smooth transition.
Simply call (800) 445-5747 or (925) 820-8300, Monday to Friday, 8 a.m. to 5 p.m., choose option 2, and tell your Customer Service Representative that you would like help to switch to a different Hill Physicians Provider.
3. Schedule Your First Visit
After you switch your PCP your Health Plan may issue a new identification card indicating your choice of a new physician. However, you don't have to wait to receive that to schedule your first visit. Using the contact information found in our Physician Directory, call and schedule your first appointment.
