Enrollment Form Help
When you want to find a new Primary Care Provider or change your doctor most HMO's ask you to fill out an Enrollment/Change Form. On most of these forms there is an area where your HMO asks you to indicate your Primary Care Provider or Personal Physician's name and/or identification number.
Finding the number for your physician can be difficult. So, we've put together some instructions on how to find this important information specific to your health plan online. First, use our online directory to find your preferred doctor. Write down his/her name or print their listing for easy reference. Next, you'll need to visit your health plan's website to look up that doctor's ID number. These tip sheets will help.
