A Patient Who Once Believed Change Was Out of Reach
One patient’s story stays with Mohini in particular. When she first connected with him, he was overwhelmed by his diagnosis and unsure where to start. He struggled with checking his blood sugar, wasn’t confident about his medication routine, and often felt confused about what he should be eating.
Early on, he told her he didn’t think he’d ever achieve the A1C levels doctors often recommend. “He said he’d never be one of those people who got their A1C down to a 7,” Mohini recalled. “He genuinely didn’t think it was possible.”
Working with his primary care doctor, Mohini helped adjust his medications and created a schedule he felt capable of sticking to. Their conversations were steady and practical — small steps, realistic goals, and strategies tailored to his day-to-day life.
As the months went by, his numbers began to change. His blood sugar stabilized. His weight came down. He started to feel more in control. Two years into their work together, the patient who once believed better numbers were out of reach recorded an A1C of 6.1. Mohini described him as “excited and proud — and motivated to keep going.”

“People succeed when they have someone walking the journey with them,” she said. “When they feel seen and supported, things start to shift.”
