Patient Experience Surveys

​​Hill Physicians wants to make sure you get the best care from our providers. Our patient experience surveys help us better understand how we can improve your care experience and give us the chance to recognize our providers for great care. You are not required to take these surveys, but we encourage you to do so as your thoughts matter to us. Please refer to the FAQs below for more information.

How will I receive a survey?

Surveys are sent two to three weeks after your appointment with a Hill Physicians provider. We use several outreach methods, so you may receive:

  • A phone call to a number you have provided.
  • A text message to a number you have provided.
  • An email to an email address you have provided.
  • A letter in the mail.

If you have seen more than one Hill Physicians provider for other appointments, you may receive additional surveys. To opt-out of these surveys, please follow the instructions at the bottom of this page.

Are my responses confidential?

Yes. We use a third-party vendor, NRC Health, to ensure all responses remain confidential. All of your answers are protected under the Health Insurance Portability and Accountability Act (HIPAA). 

What is on the survey?

Each question is specially designed to measure your experience with your healthcare provider. The questions are set by the Consumer Assessment of Healthcare Providers and Systems (CAHPS), which is run by the Agency for Healthcare Research and Quality (AHRQ), a division of the U.S. Department of Health and Human Services. To learn more about the questions, visit their website.

How will Hill Physicians use the feedback collected from these surveys?

Your participation allows us to assess the quality of the care we are providing and gives you an opportunity to make your voice heard. We may share comments from the surveys to recognize positive experiences or intervene as part of our efforts to make things better. To file a formal complaint, please contact your insurance company with the number provided on your personal card. 

Opt-Out Instructions

This outreach about your physician office visit may occur (1) by telephone at a number you have provided, including a wireless telephone number, (2) by text message at a number you have provided, (3) by email using an email address you have provided or (4) by a mailed letter to an address you have provided.
