What is a Patient and Family Advisory Council (PFAC)?

A Patient and Family Advisory Council is a forum between patients, families, clinicians, and other team members, working together to bring about change to enhance the care experience. Through this collaborative partnership, the PFAC will give guidance and strengthen the relationship between the community and the practice.

Who is eligible to participate?

A current patient or family member of someone in care by a Hill Physician Practice.

How frequently does the PFAC meet? How long are the meetings?

Meetings are held the last Saturday of the month, at 10 am excluding the month of December. Members are expected to attend a minimum of six meetings annually. The meetings are 1.0 hours maximum in length.
