Manage a Family Member’s MyHillChart

This is your starting point for requesting proxy access to MyHillChart. Proxy access lets a parent, guardian, or authorized representative manage a patient’s health information on their behalf. It applies to parents and guardians of minor patients, and to authorized representatives of patients age 12 and older. Eligibility and access levels are based on the patient’s age.

What Access Includes

The type of information you can access depends on the patient’s age.

Ages 0-11: Full Access

  • Message the care team Send messages on your child’s behalf.
  • View health records See visit notes, history, and documents.
  • View test results See lab and diagnostic results.
  • Schedule appointments Book and manage your child’s appointments.
  • Request medication refills Submit refill requests on your child’s behalf.

Ages 12-17: Limited Access*

  • Message the care team Send messages on your child’s behalf.
  • Appointment requests Book and manage your child’s appointments.
  • Immunization records See your child’s vaccination history.
  • Allergy records See your child’s documented allergies.

*Some records may be restricted for patients ages 12-17 under California privacy law.

How to Complete and Submit Your Proxy Form

  1. Download the right form for the patient’s age group by clicking the button below.

  2. Fill out all sections, including your information and the patient’s information.
  3. Email the completed form and a copy of a valid photo ID (driver’s license, state ID, or passport) to customerservice@hpmg.com.
  4. Our customer service team will follow up to confirm your identity and verify your information before access is granted.
  5. Once approved, you’ll get instructions on how to connect to the patient’s account.
